ARA-290 Research Overview (also known as Cibinetide, Hemoregulin, ARA 290)
A small peptide derived from the non-hematopoietic region of erythropoietin (EPO) that targets the innate repair receptor (IRR) without activating red blood cell production pathways. Studied for its ability to modulate inflammatory signaling, support nerve comfort, and regulate autoimmune-related irritation.
What Is ARA-290?
ARA-290 is a synthetic 11-amino-acid cyclic peptide designed to specifically activate the tissue-protective receptor (TPR) — a heterodimeric form of the erythropoietin receptor — without stimulating erythropoiesis. It was engineered to separate erythropoietin's pleiotropic tissue-protective properties from its hematopoietic effects.
The Tissue-Protective Erythropoietin Receptor
EPO also activates a distinct receptor complex — a heterodimer combining the EPO receptor with the common beta chain (βcR, CD131) — expressed on neurons, cardiac cells, kidney cells, and immune cells. This TPR mediates cytoprotective, anti-apoptotic, and anti-inflammatory effects distinct from erythropoiesis. ARA-290 was designed to activate this receptor selectively.
Neuropathic Pain Research
The most advanced research application for ARA-290 is small fiber neuropathy — a painful condition common in diabetes, sarcoidosis, and autoimmune conditions. Clinical trials have demonstrated statistically significant pain reduction in small fiber neuropathy patients, with concurrent evidence of small fiber nerve regeneration as measured by skin biopsy — one of the first compounds to show both symptomatic relief and evidence of actual nerve repair in this condition.

Combined tissue repair mechanisms may enhance recovery from injury - Ara 290 provides anti-inflammatory effects while TB-500 promotes cellular migration and angiogenesis
No known interactions between IRR activation and thymosin pathways, both work through different mechanisms for tissue protection and repair
Clinical trials exclude recent EPO use within 2 months due to potential receptor interference and unclear combined effects on hematopoiesis
Both affect cellular metabolism and stress response - monitor for additive anti-inflammatory effects and potential enhanced tissue protection
Clinical trials included patients on antidiabetic medications including GLP-1 agonists with no adverse interactions, may provide complementary metabolic benefits
Both affect tissue repair and growth pathways - combination may enhance effects but requires monitoring for potential excessive growth factor activity
Clinical protocols require 6-month washout from anti-TNF therapy before Ara 290 to avoid potential immune system interactions
Reported Research Timeline
01Week 1–2 (reported in cited studies): initial anti-inflammatory effects, potential mild improvement in pain symptoms
02Week 2–4 (reported in cited studies): progressive nerve regeneration, improved tissue healing markers, enhanced wound healing if applicable
03Week 4–6 (reported in cited studies): peak therapeutic effects, maximum nerve fiber density improvements, sustained pain relief
04Month 2–6 (reported in cited studies): long-lasting benefits continue due to "molecular switch" effect, sustained tissue protection
